Registering with a GP practice

To register permanently, you’ll need to complete a registration form. You can:

  • Download a registration form, complete it, and then email it to the GP practice. Please download and print the Pathhead Medical Centre New Patient Questionnaire. Make sure to fully complete this form for each family member. 
  • Ask the reception team for the registration forms. You can complete these in the practice or take them home. They must be signed by each adult. Please hand back to the receptionist. 

When you register with us, it’s recommended that you provide information to confirm your:

  • identity
  • address

This means you can be matched to your existing NHS records. You shouldn’t be delayed or refused registration at a GP practice if you can’t provide identification or proof of address.

Anybody in Scotland can access primary care services at a GP practice without charge.

Registering as a Temporary Resident

If you are visiting in the area and you need urgent medical care you can register as a temporary resident.  A temporary registration is valid for up to 3 months.   You must also be within our catchment area to register as a temporary resident, this can be checked at NHS inform

To register you will need to complete the one page form at our reception desk, or alternatively once you have spoken to our Reception team on 01875320302 you may complete the online form.
